Death in Custody Forms

PLEASE NOTE THAT THE DEATH IN CUSTODY REPORT FORM IS NOW DUE ONLY TWICE PER CALENDAR YEAR, FOR REPORTING PERIOD OF JANUARY 1^ST - JUNE 30^TH, AND JULY 1^ST - DECEMBER 31^ST. THOSE REPORTS ARE DUE BY EITHER AUGUST 1^ST FOR THE FIRST HALF REPORT OR FEBRUARY 1^ST FOR THE SECOND HALF REPORT.

You may fax to 785-296-0927 the completed fill able forms to the Sentencing Commission. You will need Adobe Acrobat Reader to access these files, which may be downloaded off this page. The forms may be emailed to: Office@sentencing.ks.gov

If you have difficulty completing the form AND there are no reported deaths, you may simply send an email stating that for the particular period there are no deaths to report for your agency. The email will serve as documentation.
