OPEN RECORDS POLICY



Freedom of Information Officer/Official Custodian: Douglas "Mack" Smith, Executive Secretary

Agency's Working Hours: 8 a.m. to 12 noon and 12:30 p.m. to 4:30 p.m. Monday - Friday, except for designated state holidays and when it is necessary for employees to be out of the office.

Cost of Copies: 25¢ per copy page plus postage

In order for individual(s) wishing to view or receive any Mortuary Arts Board records deemed open to the public, it shall be necessary for those individual(s) to:

1) contact the board's official custodian of records either by telephone, mail, fax or e-mail;

2) disclose the name and address of the person wishing to view or receive records;

3) when contacting the Mortuary Arts Board's Freedom of Information Officer/Official Custodian, arrange for an appointment time (if records are to be viewed), and

4) provide a positive form of identification before viewing records.
 

Due to the small size of the office, the Mortuary Arts Board's Freedom of Information Officer/Official Custodian may not be available on any given day.
