By clicking on the following link Individual Licensees, Establishment/Branch Establishments and Crematories can pay their fees on-line HOWEVER, this is just a payment method only!!  There is also a "convenience user fee" that is in addition to  your license fees due. You still must submit all  your information by mail, ie:  signed renewal form, continuing education cards/certificates if an individual. If you are an establishment/branch establishment or crematory, you will have to submit your renewal, roster application, and pre-needs by mail. The on-line renewal link is:  KanPay

The above is a list of all funeral homes and crematories with refrigeration units.
Licensees can provide us with a change of address by filling out the following  Change of Address Form  which will be emailed directly to us so  we can update our files.
Licensees can provide us with a change of address by filling out the following Change of Address Form.pdf in Adobe Acrobat Reader pdf format which will allow you to print, complete and mail to our office so our files can be updated.

NOTE: In order to reinstate any dropped license, it is necessary to pay all past-due fees plus a penalty in the amount of the current renewal fee and to meet all current Kansas licensure requirements. All past due continuing education hours must be made up within one year of reinstatement in order to be licensed on an active basis.
