RENEWAL
APPLICATION: A renewal form and a continuing education form will be mailed
to you the first week of October in odd-numbered years. Please type or print
using black ink. THE FORM SHOULD BE SIGNED AND DATED and postmarked by December
1 of the expiration year of your license. Please review the pre-printed
information on the renewal form and make any revisions that are applicable.
FEE: The biennial license renewal fee for dental hygienists is $150. CONTINUING EDUCATION: For any continuing education that was not physically attended the dental hygenist shall submit evidence of satisfactory completion. Each course shall have been completed in the 24-month period immediately preceding the date of expiration of the dental hygiene license for a total of 30 hours. Any hygienist licensed less than 24 months, but more than twelve months, from the expiration date of the license is only required to submit 15 hours of continuing education. Any hygienist licensed less than twelve months from the expiration date of the license is exempt from continuing requirements for their first renewal cycle. CANCELLATION: Pursuant to K.S.A. 65-1431(e)(1) "Any license granted under authority of this act shall automatically be canceled if the holder thereof fails to apply for renewal within a period of one month from December 1 of every other year." Please notify this office in writing if you wish to cancel your license per your request. REINSTATEMENT: Pursuant to K.S.A. 65-1431(e)(2) "Any licensee whose license is automatically canceled by reason of failure, neglect or refusal to secure the renewal certificate may be reinstated by the board at any time within one month from the date of the automatic cancellation of such license, upon payment of the biennial renewal fee and upon proof that such licensee has satisfactorily completed a program of continuing education required by the board or at any time within two months from the date of the automatic cancellation of the license upon payment of the biennial renewal fee and a penalty fee of $500 and upon proof that such licensee has satisfactorily completed a program of continuing education required by the board." Any licensee
that fails to meet the requirements for reinstatement by March 1 will be
required to file an application for licensure and comply with the requirements
