Article 1. Client Assessment Referral and Evaluation (CARE) Program


120-1-2. Data collection form used by department on aging.
Before any individual may be admitted to a nursing facility, a certified assessor with the client assessment, referral, and evaluation (CARE) program of the department on aging shall evaluate and refer the individual according to the data collection form approved by the secretary of aging and adopted by the health care data governing board.
